What to bring to all appointments:
The Coconino county health code requires your consent to every new procedure, therefore you must have a valid government issued form of ID with you at the time of your appointment. We cannot use expired forms of ID.
Acceptable forms of identification include:
State-Issued Drivers License or Instruction Permit
State-Issued ID Card
Passport or Passport Card
1-94 form with photograph
Photographs of IDs will not be accepted.
For minors getting pierced or tattooed, you must have the following with you;
You must be accompanied by a birth parent or legal guardian.
If you are bringing your guardian, you must bring the official court custody documents.
Both adult and child must have an acceptable form of ID from the above list to identify the both parties.
You must bring your actual birth certificate or full-size printed copy to prove parent-ship.
Photographs of IDs or birth certificates will not be accepted.
